Chennai, In another landmark Pediatric surgery, Dr. Mehta’s Hospitals, the best multi-speciality hospital in Chennai, successfully treated an 11-month-old child with an extremely rare and complex condition, intra-thoracic appendicular perforation with gangrene associated with a giant Morgagni hernia.

The child presented with vomiting and abdominal distension, but X-ray findings revealed a giant Morgagni hernia. Intra-operatively, surgeons discovered pus within the thoracic cavity due to appendicular perforation with gangrene. The surgical team performed an appendectomy along with successful anatomical repair of the Morgagni hernia.

Despite the diagnostic and intraoperative challenges, the surgery was a success. The child made a swift recovery, has shown excellent long-term outcomes with no recurrence, and remains well on periodic follow-up.

A major challenge in this case was that while mesh repair is ideal for a giant Morgagni hernia, the presence of appendicular perforation with infection made mesh placement unsuitable. The surgical team opted for a challenging yet precise anatomical repair, ensuring complete resolution.

This is the youngest reported child (11 months old) in India with this rare and complex combination of diagnoses to be successfully treated. The case highlights the expertise of Dr. Mehta’s Hospitals’ Pediatric Surgery Department in managing rare and high-risk Pediatric surgical conditions.
